Course Description

Patients facing advanced illness eventually come to a point when certain medical treatments no longer contribute to their quality of life, and they may desire a transition to a comfort focused approach to their care. Skilled clinicians help patients through this transition by engaging in a transition conversation at the appropriate time. This course will provide clinicians with the skills to:

  • Identify when patients may be ready for a transitions conversation
  • Properly structure transitions conversations
